Hospital to Home or Care Community: A Discharge Planning Guide
Hospital discharge is often where families get blindsided. Discharge planners move fast, options aren't always explained, and your parent may not be ready to go home. This guide walks through what's actually on the table and how to make a calm choice in a compressed window.
What Discharge Planners Will Offer
Home with home health: nurse and therapy visits a few times per week, typically Medicare-covered for a defined episode.
Skilled nursing facility (SNF) for short-stay rehab: Medicare-covered for up to 100 days after a qualifying 3-day hospital admission, with day 1–20 typically fully covered and day 21–100 requiring a copay.
Outpatient therapy: if your parent can travel.
What they usually don't mention: residential assisted living, boutique assisted living, or memory care as a longer-term option.
Questions to Ask Before Discharge
What is the actual functional goal — can my parent realistically live alone safely?
Will Medicare cover a short-stay rehab, and at which facilities?
What home services has the hospital arranged, and how soon do they start?
Is a follow-up with primary care or specialists already scheduled?
When Short-Stay Rehab Makes Sense
After a fall, stroke, surgery, or significant hospitalization where therapy can meaningfully improve function.
Short-stay rehab is typically 14–30 days, sometimes longer. Use this window to also explore longer-term options before discharge from rehab.
When to Skip Home and Go Directly to a Care Community
If your parent can't safely manage at home alone and family can't be present 24/7.
If a previous discharge ended in readmission within 30 days.
If cognitive change is now significant enough that managing medications, meals, and safety independently isn't realistic.
