Disclosure Notice

Arizona law requires that we provide you with this disclosure notice.

Mentoring Our Moments provides referral support for senior living options. If you move into one of the referred homes or communities, Mentoring Our Moments may be paid by that home or community in connection with the referral.

The fee may be described as a dollar amount or as a percentage of the first month's rent and care charges. Compensation may vary and, when not yet determined, a good-faith estimate may be provided.

Mentoring Our Moments may have a current business relationship, common ownership or control, or another financial, business, management, or familial relationship with one or more of the homes or communities to which referrals are made.

You have the right to terminate our services at any time by providing written or electronic notice.

Our Commitment to You

Our role is to help families find the best-fit next step with clarity, compassion, and integrity. Recommendations are guided by the client's needs, preferences, care concerns, location, and overall situation.

No Cost to Families

Families are never charged for our referral support, guidance, or advocacy. Compensation, where applicable, is paid by the community or home — not by the family.

Intake Acknowledgment

As part of our intake process, families are asked to acknowledge the following:

"I acknowledge that Mentoring Our Moments may receive compensation from select partner homes or communities, and that one or more referred options may involve a business or financial relationship disclosed by Mentoring Our Moments."
